A pharmacy team member will assist you with your consent form and insurance information upon arrival. Completing a consent form before you arrive will save you time. Things to remember before your visit

Please wear a face covering during the immunization

Bring both your prescription and medical insurance cards with you

Wear clothing that provides easy access to your upper arm

If possible bring a completed consent form with you

CDC recommends waiting 15 minutes after receiving an immunization. Feel free to wait in a designated parking place then enjoy the rest of your day

Our pharmacists are working to keep you safe!

Patient screening for COVID-19 symptoms prior to providing immunizatons.
Personal protective equipment(PPE) for all pharmacy associates, including face shields.
Hand hygiene practiced before and after immunizations, including changing gloves between patients.
Monitoring of COVID transmission and prevalence in the local area to adapt PPE requirements if nessessary.
Maintaning social distancing throughout the immunization process.
